medical release and hold harmless form info
 here's
what needs to be on the medical release form... though there's probably
more that i don't know about, so if you think of anything just go ahead
and include it...

- if she can swim.  if she can't they should enclose info about why not.
- include info about what daily medicines are needed.  who can give
them.  etc.
- allergies (food and medical).  they must write that there are NO
allergies or what they are.
- emergency numbers
- general medical info (this is where i don't really know what
specifically we need to know)
- medical insurance info (? do we need this)

here's the sample hold harmless form
(was going to send it to you through campus mail, but now i have to type
it in, so sorry for typos or bad format etc...):

HOLD HARMLESS RELEASE

                Date:__________

Name of Person Giving Release: ____________

(we should add, name of daugher: ___________ and above maybe name of
parent)

Party Released: Swarthmore College, its agents and employees including
administration, faculty, and staff.

(we need to add something about the "coordinators of SCLP" -
except it'll have a new name, though for now you should just use SCLP, or 
perhaps Summer Community Learning Program since this is just a first
draft anyway)

Release:  I release and give up all claims I now have or may have in the
future against the Party Released arising out of my participation in
(SCLP).

I understand that the Release applies to all claims for injuries--illness
and any other damages suffered by me.

Binding:  This Release binds me, my heirs and personal representatives
(maybe add daughter?).  I understand that it benefits the heirs, personal
representatives or successors and assigns of the Party Released.


Signing:  Before signing my name to this Release, I state that:
        1.  I have read it.
        2.  I understand it and know that I am giving up important rights.
        3.  I sign it freely as my own act and deed, and
        4.  I intent to be legally bound by it.

ok, other things that need to be added are:
- something about the Sex/HIV/AIDS presentation that was presented to them
at the info session that we're going to hold being fine with them.
- that it's ok and they understand that if their daughter asks us
questions of any nature (sexual, hiv related, etc. etc. ) that it will be
our policy to answer it.
- that we're allowed to administer medicines/ treatment
- that we're allowed to take public transit (though we don't really know
if we're doing that yet).
- that they understand that their daughter may/will be swimming and that
that's fine.


that's it for the hold harmless.
if you have extra time and feel ambitious, it would also be great if you'd
write an introductory letter, like one that we'd send out to all of the
campus when we're trying to recruit girls, that includes info on:
- what the program is
- what our goals are (this and above can be pretty brief)
- when it is (maybe june 15th - june15th + 2 weeks and it's during the
day with perhaps one overnight)
- that they'll need to sign a hold harmless form and medical release form.
- that we'll take the first 8 girls who are daughters of faculty and the
first 8 who are daughters of staff and put the next 10 of each on a
waiting list if we get too many applicants.
- that to apply all they have to do e-mail us or send something to us
through campus mail with the name of the girl, their year, their relation
to someone on campus, plus anything else you think of (put something on
the bottom of the form that they can tear off and send back that has this
info on it).
- that the program is $20 (to cover the cost of the snacks), at least
that's what i currently think.

that last thing is really only if you're feeling ambitious and want to, it
doesn't need to be done by the end of break.
